Patient Notice – FMLA Paperwork Administrative Fee

At Rehoboth Healthcare, we are committed to supporting our patients with the documentation they may need for employment or medical leave purposes. Please note that completion of Family and Medical Leave Act (FMLA) forms requires additional administrative time outside of your clinical appointment.

• The standard fee for FMLA paperwork is $100.
• Fees may be higher depending on the complexity of the forms or if additional documentation is requested.
• Payment is due at the time the forms are submitted for completion.

We appreciate your understanding as these administrative services are not covered by insurance.
